Guidelines for State and Chapter Yearbooks
1. The cover - page should contain the insignia of the National Society, United States Daughters of 1812. Nothing shall be printed above the insignia. The chapter name and state where the chapter is located or the state society should appear on the cover - page.

2. A title page that includes the motto, colors and official
flower of the National Society. The EIN # for individual Chapters and the state
EIN # should be printed on this page. The date of organization of the National
Society as well as the date of organization of the state society or the
individual chapter organization date. The
title page must contain the current year.

3. One or more pages that contain the Purpose of the National Society, the Ritual, the American’s Creed , the Pledge of Allegiance to the Flag of the United States of America and the Salute to the Flag of the 1812.

4. Pages that list the current National, State and Chapter officers with current addresses, phone numbers and emails for the National and State officers. State yearbooks should list their Honorary State Presidents.

5. State yearbooks should contain pages with the name and contact info for National and State Chairman. Chapters may just list name of Chapter chairman.

6. A list of important dates that include National, State and Chapter meeting dates. The time and location of each should be listed.

7. An up to date alphabetical chapter roster that includes the name of the member, spouse, address, phone number, name of ancestor and state served from. Email if applicable. The National number should be listed as well. Example: Jones, Mary Smith (Mrs. John)

8. The address, phone number, email and website address for National Headquarters should be listed.
